Patients Who Are Carers/Cared For
If you are a carer for someone else, or you are cared for by someone else, please let us know so we can help support you. A carers registration form can be obtained from our waiting room or from reception. You will then receive newsletters 3 times a year from Carers Support.

NHS Surrey in partnership with Action for Carers (Surrey) and Surrey County Council have developed a ‘Carers health needs survey’ to help understand the impact that caring has on our patients health. If you are a carer, please complete the survey at http://www.surveymonkey.com/s/surreycarers . The survey will close on 10th April 2011.
