New Patient Registration

We welcome new patient registrations to Medwyn Surgery. Anyone who lives within our catchment area can register with the practice. To register online click on the 'online registration' tab above (you will need to sign the completed form when you come into the surgery for the first time). If you want to register the whole family you will need to submit a separate form for each family member.

When registering, please ensure that you bring the following documentation with you to the surgery (this does not apply to children):-

• Photographic ID
• Recent proof of address (utility bill or bank statement)

Online registration with the practice

If you wish to register with the surgery click on the link below to open the registration form. When you have completed all of the details, click on the "Send" button to mail this to us. When you visit the surgery for the first time you will be asked to sign the form to confirm the details are correct.

As part of the registration process, you are required to complete the online registration form AND the medical questionnaire below. This is because it can take a considerable time for us to receive your medical records. There is an online version of this below which can be completed prior to you visiting the surgery in order to save time at your first appointment. You will also be asked to sign this form when you attend to confirm the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect. You will NOT be registered at the practice if you have not completed the registration form AND the medical questionnaire.
