Parking at the surgery?  Parking in the Pay & Display car park is the responsibility of the patient and if you are kept waiting for an appointment,  you will need to ensure that you top up the meter accordingly. Please speak to a receptionist and let them know you've left the surgery to top up your parking.  Unfortunately the surgery can no longer provide letters of support for any penalty ticket appeal.


Flu season is upon us flu jabs are now available. If you are 65 years old or over, have a chronic health condition or are pregnant, please call into the surgery to book your appointment.


Friends of Medwyn Quiz Night is taking place on the 10th November at Denbies Sports Hall. Tickets cost £15 per person which includes fish & chips. Book your tickets at reception.


Online registration with the practicePreRegistration

If you wish to register with the surgery click on the link below to open the registration form. When you have completed all of the details, click on the "Send" button to mail this to us. When you visit the surgery for the first time you will be asked to sign the form to confirm the details are correct.

As part of the registration process, you are required to complete the online registration form AND the medical questionnaire below. This is because it can take a considerable time for us to receive your medical records. There is an online version of this below which can be completed prior to you visiting the surgery in order to save time at your first appointment. You will also be asked to sign this form when you attend to confirm the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect. You will NOT be registered at the practice if you have not completed the registration form AND the medical questionnaire.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
